The FINANCIAL -- A plastic surgery boom started in Georgia several years ago and since then different kinds of plastic operations have risen in popularity among Georgians. The nose operation is the most demanded plastic surgery in Georgia. Alexander Kalantarov, MD PhD Plastic Surgeon of Caraps Medline, says that breast implants and liposuction also top the list of most popular plastic surgery operations in Georgia.
The Clinic of Aesthetic and Reconstructive Surgery Caraps Medline was established in 1998. It was one of the first private medical clinics in Georgia. Since 2001 the clinic has had a multi-profile status. From 1998 till today Alexander Kalantarov has been working at Caraps Medline as head of the plastic surgery department.
In terms of surgical procedures Caraps Medline offers Rhinoplasty, Face Surgery, Otoplasty, Breast Surgery, Body Contour Surgery, Liposuction, Reconstructive Surgery, Vascular Surgery, Gender Surgery and Non-Surgical Procedures.
“The most demanded procedure nowadays is a nose operation, followed by liposuction, breast augmentation, stomach operation and facial surgery. Facial surgery includes eyelids, ear and eyebrow lift and generally making the whole face look better. Botox injection, gel injections, and lip enlargement are quite popular procedures as well. These are in total the most popular operations among our clients, but first place belongs to nose operations, liposuction and stomach operations,” Kalantarov said.
Kalantarov says 95% of the clinic’s clients are women.
“Nose correction, liposuction and stomach surgery are also demanded by men. The average age of our patients is from 20 to 50 years. These are the most active years, when a person starts their private life and wants to make it perfect and beautiful as quickly as possible,” Kalantarov said.
“Sometimes people realize that their future business and career prospects will depend on their appearance, so they visit us with different demands to change something about their looks. Women who are more than 50 years old mostly ask for a face lift operation, or rejuvenation, in order to prolong their career in theatre or film. They want to make the most active period of their life last longer,” Kalantarov said.
A nose operation costs 1,500 GEL and goes up to 10,000 GEL, depending on the demand and difficulty of the operation. Ordinary operations for nose reconstruction costs around 1,500-3,500 GEL.
The most popular breast enlargement operation with implants costs 6,000 GEL.
The cost of liposuction ranges from 1,000 to 3,000 GEL.
“There exist different methods of plastic surgery in Georgia, some of the surgeons use old methods and some use modern methods. Plastic surgeons need to learn throughout their lives, they should read modern magazines, visit podiums, attend conferences, and see the newest tendencies in the world to remain at the top of their sphere of practice,” Kalantarov said.
“Breast operation methods have changed. All surgeons all around the world are now trying to reach a maximal result with minimal surgical interference. It is the aspiration of any plastic surgery doctor to reach the maximal result with minimal trauma and minimal wound,” Kalantarov said.
The most expensive plastic operation is face surgery.
“The injection of Botox or gel are quite easy procedures. Botox and gel are valid for 6-8 months and then a person returns to their pre operation state.
“We use silicon for 80-90% of breast operations. In many cases it’s impossible to get a beautiful breast shape without implants,” Kalantarov said.
“Nowadays there are 5-6 euro standard companies in the world which produce the implants. These companies produce modern implants and are competing each other like car producing companies compete each other. Our clinic uses French implants, which are of very high quality and from an acceptable company for us to cooperate with. All of the company’s implants are quite safe,” Kalantarov said.
“Aesthetic surgery is never considered a necessity. Most of the people visit us for aesthetic surgery in order to look nice. More people visit us for aesthetic operations than reconstructive ones,” Kalantarov said.
Kalantarov says that women might visit the doctor twice a year to have small procedures done.
As Kalantarov says, neither Botox nor plastic surgery existed in the past, but there were other means to make one more beautiful, and the only difference is that these means have developed considerably over time.
“Women always try to look good. Women might start using Botox from the age of 30 before the first age wrinkles appear. Botox helps to improve frown lines but it’s not able to stop age wrinkles. Botox is quite safe and if women use it before the age of 50 it is very good,” Kalantarov said.

Face Surgery:
Botox Injections
Before Botox was even approved by the FDA for cosmetic use, Botox injections were the third most popular non-surgical procedure with 855,846 patients in 2001. 55% of all Botox injection patients were between the ages of 35-50. Botox injections were FDA approved in April 2002 for cosmetic facial rejuvenation as it has been proven to smooth and even eliminate facial wrinkles such as forehead lines, frown lines, crow’s feet and neck bands. That year, Botox injections was the top non-surgical cosmetic procedure with 1,123,510 cosmetic Botox injections performed! Botox is also used in conjunction with forehead lifts, facelifts, and eyelid surgery. Botox injections work by relaxing targeted muscles. Typically the effects of Botox injections last three to six months and must be repeated to maintain results.
Earlobe Surgery
Earlobe surgery can be done in patients whose earlobes have become stretched or elongated or have actually split. This is often secondary to trauma or to wearing heavy earrings over a long period of time.
Midface Lift
This procedure is designed to correct hollowing beneath the lower eyelids and deep nasolabial folds which run from the base of the nose to the outer corners of the mouth. It creates a more youthful appearance by smoothing the transition between the lower eyelids and upper cheek region. If improvement in the neck or jowl region is desired, a Midface Lift can be combined with a more standard facelift.
Facelift
A rhytidectomy or facelift is designed to restore a more youthful appearance to the neck and jowl region by removing major wrinkles and sagging skin. If there is excess fat in the neck or chin region, this can simultaneously be addressed with liposuction or direct excision. A facelift can improve a tired facial appearance by smoothing the skin and re-establishing defined contours along the neck and jaw line. It is often done in combination with other procedures including a browlift and eyelid surgery to create an overall harmonious, youthful result.
Brow Lift
A browlift is done to raise the eyebrows and reduce ridges and furrows on the forehead, thus creating a smoother, more youthful appearance. This procedure is typically done with other facial surgeries, namely those on the eyelids. The originally described browlift involved an incision in the hairline extending from one ear to the other. More typically today, the browlift is performed endoscopically using 3-5 smaller incisions in the scalp and specialized instruments to elevate the skin and muscles of the forehead and re-suspend them.
Oculoplastic Procedures
Oculoplastics is the surgery of eyelids and orbit. Primary cosmetic eyelid procedures as well as revision eyelid procedures to correct complications after eyelid surgery are performed on a regular basis.
Eyelid Lift
Age, sun exposure and hereditary factors all contribute to wrinkles, deep lines and puffiness around the eyes. This can create a tired or aged appearance. An eyelid lift, or blepharoplasty, is designed to restore a more youthful and rested appearance to your eyes. It can also improve your scope of vision if you have excessive upper eyelid skin that interferes with peripheral or superior vision. A blepharoplasty can involve the upper lids, the lower lids, or both.
Breast Surgery:
Breast Augmentation
Breast enlargement, or augmentation mammoplasty, enhances the body contour of a woman who is unhappy with her breast size. This procedure can also correct a reduction in breast volume after pregnancy or balance a difference in breast size. In certain cases where the breasts have sagging or droopiness, a simultaneous breast lift may be necessary.
Revision Breast Augmentation
If you have had a previous breast implant surgery and desire a size change, a revision augmentation is necessary. Additionally a revision is the only means of correcting problems associated with rippling, implant rupture or excess scar formation termed capsular contracture. Revisional breast surgery attempts to restore breasts that are soft and symmetrical.
Breast Lift
A mastopexy or breast lift will raise and reshape a woman’s breasts. Over the years breasts can lose their shape and firmness due to pregnancy, nursing, weight fluctuations and gravity. A breast lift, often done in conjunction with a breast augmentation, can eliminate this inevitable sagging. If the nipple areolar complex has become enlarged, it can be restored to a smaller size at a preferred height on the breast.
Breast Reduction
Breast reduction, or reduction mammoplasty, is for a woman experiencing health problems and/or extreme self-consciousness associated with very large, heavy breasts. Medical problems associated with large breasts include back and neck pain caused by the excessive weight, skin irritation, bra strap indentations, poor posture, and interference with normal daily activities such as exercise. The surgery removes fat, glandular tissue, and skin from the breasts, making them smaller, lighter, and firmer. It can also reduce the size of the areola, the darker skin surrounding the nipple. This can alleviate the health problems associated with large breasts and improve a woman’s confidence in her appearance. Reduction mammaplasty is often covered by insurance.
Male Breast Reduction
Male breast reduction is done for men with gynecomastia, a condition of male breast overdevelopment. The procedure removes fat and/or glandular tissue from the breasts, and in extreme cases removes excess skin, resulting in a chest that is flatter, firmer, and better contoured. The surgery often involves liposuction as well as some component of direct breast excision done through a semicircular incision around the areola. In cases of skin excess, additional incisions will need to be made for skin removal.
Breast Reconstruction
Breast reconstruction attempts to recreate a breast mound in women who have had their breasts altered or removed by breast cancer surgery. The goal is to rebuild a natural looking breast with symmetry to the other breast. Depending on the circumstances, breast reconstruction can be performed immediately after a mastectomy or as a delayed procedure, anywhere from several weeks to years following mastectomy. Breast reconstruction typically requires more than one operative procedure, with additional surgeries to optimize symmetry or rebuild a nipple areolar complex.
Body Contouring after Massive Weight Loss
Significant weight loss or gastric bypass surgery can leave excess skin. A circumferential body lift can eliminate this skin redundancy and improve contours in the abdomen, buttock, thighs and back. The surgery involves removal of the excess skin rolls to leave an area smoother and flatter.
