OLYMPIA — The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Benton County
In September 2023 the Nursing Assistant Program suspended the registered nursing assistant license of Yesenia Adi Chavez (NA60241434) pending further legal action. Charges state that Chavez neglected patients in an adult family home she owned. The charges included neglect to wound care leading to a patient death and a patient discharged to end of life care.
Cowlitz County
In September 2023 the secretary of health indefinitely suspended the certified nursing assistant license of Angela M. Lasseigne (NC10060601). Prior to any petition for reinstatement, Lasseigne must complete 12 hours of continuing education in the areas of patient rights, law and ethics, and theft awareness. She must also pay a $1,000 fine.
Island County
In August 2023 the Board of Nursing (formerly the Nursing Commission) indefinitely suspended the licensed practical nurse credential of Julie Ann Moehring (LP60329704). Moehring failed to comply with a 2023 agreed order by withdrawing from a monitoring program and failing to comply with her monitoring contract.
King County
In September 2023 the secretary of health indefinitely suspended the medical assistant-phlebotomist license of Chelsie Marie Andrew (PC60346351). Prior to any petition for reinstatement, Andrew must contact a monitoring program and enroll if the program determines it is required. Andrew must also undergo a mental health evaluation. In September 2020, Andrew was convicted of assault and malicious mischief in relation to two 2019 incidents of physically attacking someone while intoxicated.
In July 2023 the secretary of health revoked the nursing pool license of Heritage Medical Staffing, Inc. (NR60760975). Between April 2018 and 2020, Heritage Medical Staffing, Inc. gave credentials and identity information of health care workers to unlicensed, unqualified and/or unauthorized individuals allowing them to work under valid health care credentials.
In July 2023 the secretary of health revoked the nursing pool license of Promed Alliance Medical Staffing, Inc. (NR60935499). Between April 2018 and 2020, Promed Alliance Medical Staffing, Inc. provided credentials and identity information of health care workers to unlicensed, unqualified and/or unauthorized individuals allowing them to work under valid health care credentials.
Snohomish County
In August 2023 the secretary of health indefinitely suspended the certified nursing assistant license of Lionel D. Folgado (NC61051226). Prior to any petition for reinstatement, Folgado must complete six hours of continuing education in the areas of law and ethics, communication with cognitively impaired patients, and patient rights. He must also pay a $250 fine. In January 2022, Folgado physically and verbally intimidated a patient.
